  • Weekly review of collected data with self care and health management discussion as appropriate
  • Symptom and early warning signs education and monitoring
  • In-home services with independent contractors coordinated/managed (housekeeping, laundry, food service, pet care, lawn care, grocery and personal goods delivery)
  • Quarterly medication reconciliation and education
